Job Description:
Job Title: Mental Health Counselor – Positive ChangesQualifications: Masters degree in field of mental health counseling, social work, psychology, or related human services field with at least two years work experience. Multilingual in English and Spanish, preferred. Licensure required. Experience with adolescent population preferred. Scope of Position: Duties include managing an adolescent case load, assessments, individual and group counseling, treatment planning and referral. § Gather and analyze information on clients’ presenting circumstances, social and psychiatric history, level of functioning and/or immediate needs § Develop and implement specific goals and methods in patient individual treatment plans stemming from results of assessment. § Participate in psychological related activities and treatments as a member of a multidisciplinary team. § Maintains clinical caseload § Provide individual and group counseling sessions as determined by patient needs § Provides co-occurring disorder specific services if indicated. § Consult with other disciplines (i.e. Behavioral Interventionists, School Counseling Staff, Psychiatrist, Psychologist) concerning coordination of client care. § Refers, coordinates and tracks outcomes of referrals for clients requiring services beyond scope of the program. § Implements best and evidence based treatment practices § Participate in staff meetings, in-service trainings, conferences and workshops § Participate as a team member in case management conferences § Ensures each client has appropriate discharge plans including continuing care, if indicated, and has adequate resources required to support mental health stability at the time of discharge. § Maintain clinical records in compliance with agency, local and federal guidelines. Ensure that documentation indicates compliance with timelines. § Assists with the collection of evaluation data. § Participate in Performance Improvement activities as required by the Program Coordinator. § Serves as member of Quality Improvement Team and conducts assigned quality improvement activities |
