Job Title: Family Coach – Marriage & Family; Positive ChangesQualifications: Master’s Degree in health related field or Bachelors Degree with a specialty in family therapy and two years supervised work experience in family therapy may substitute for the specialization in graduate school; Ability to understand and implement evidence based practice including brief strategic counseling strategies; Excellent verbal and written communication skills; Critical thinking and creative problem solving skills; Ability to work independently; High level of initiative. Scope of Position: Provides family counseling to individual families. Provides group family education and support groups § Collect information about clients, using techniques such as interviewing, discussion, and observation. § Counsel clients on concerns such as unsatisfactory relationships, divorce and separation, child rearing, home management, and financial difficulties. § Determine whether clients should be counseled or referred to other specialists in such fields as medicine, psychiatry and legal aid. § Develop and implement individualized family care plans addressing family relationship problems. § Develop care plans based on brief strategic family counseling models that are strength-based, measurable and include individual family goals. § Encourage individuals and family members to develop and use skills and strategies for confronting their problems in a constructive manner § Maintain case files that include activities, progress notes, evaluations, and recommendations. § Confer with other counselors in order to analyze individual cases and to coordinate counseling services § Contact doctors, schools, social workers, juvenile counselors, law enforcement personnel and others to gather information in order to make recommendations to courts for the resolution of child custody or visitation disputes § Follow up on results of counseling programs and clients' adjustments in order to determine effectiveness of programs § May provide family counseling and treatment services to inmates participating in substance abuse or re-entry programs. § Provide instructions to clients on how to obtain help with legal, financial, and other personal issues. § Writes evaluations of parents and children for use by courts § Confer with clients in order to develop plans for post-treatment activities. § Participate in Performance Improvement activities as required by the Program Coordinator. Serves as member of Quality Improvement Team and conducts assigned quality improvement activities
