Job Description:
Job Title: Re-entry CoordinatorQualifications: Bachelor’s degree in field of job training, counseling, social work, psychology, or related human services field or equivalent experience. Multilingual in English and Spanish, preferred. Experience with criminal justice population preferred Scope of Position: Duties include managing and providing re-entry services including development of a network of job opportunities for persons recently released from incarcerations; provides behavioral interventions, job readiness and training, and family reunification services to persons incarcerated and recently released from the correctional system in order to ensure successful reentry into the community. § Adheres to facility safety and security standards § Serve as liaison to correctional facility staff § Ensure program processes, activities and staff are compliant with correctional facility policies § Develops and promotes network of local job opportunities for program participants § Supervises Job Coach and Group Facilitators § Provides training, performance improvement and disciplinary actions with program staff. § Conducts assessments including Comprehensive Intake Assessment, Drug and Alcohol Assessment, Mental Health Screening, Offender Reintegration Scale (ORS) and Criminal Thinking Scale (CTS) and remedial education assessment § Develops and monitors client centered Re-entry plan based on assessment and client goals, objectives and completion criteria § Implements behavioral interventions § Plans and ensures implementation of family reunification services. § Plans and ensures job readiness and job search services § Coordinate referrals with community based providers § Plans and implement community restitution modality in the form of client participation in community service activities § Identifies and refers clients to community based remedial education services if indicated § Assists in medication education and promotes medication compliance. § Lead staff meetings, in-service trainings, conferences and workshops § Maintain clinical records in compliance with agency, local and federal guidelines. Ensure that documentation indicates compliance with timelines. § Completes program staff timesheets § Monitors and controls expenditures related to program budget § Compiles reports as directed by Clinical Supervisor. § Participate in Performance Improvement activities as required by the Clinical Services Coordinator. § Coordinate efforts with service providers in the community. § Serves as member of Quality Improvement Team and conducts assigned quality improvement activities§ Assists with the collection of evaluation data. |
