Job Description:

The Family Outreach Educator will enroll clients, educate about health risk reduction, disseminate incentives, track clients to follow up on referrals, schedule health appointments, and various other activities.  The FOE will also do outreach and make presentations at designated Community events, neighborhood centers, businesses and organizations.  


Skills Required

Computer competency, strict confidentiality, strong people skills and ability to work independently as well as collaboratively. Valid driver's license, H.S. diploma (college or technical degree preferred)
